US Airways computer systems leave passengers stranded
It was all over the news:

Computers were down for hours on Friday leaving people unable to check in, print boarding passes, even the website was down. This job posting:

shows that they use Microsoft technologies which would certainly cause reliability problems. Although I understand in this case the problem was a datacenter power failure. Apparently their backup batteries and generators didn't work as they should. Windows programmers, incompetent datacenter staff, birds of a feather...
